Depression Program

The Depression Program is based upon the Case Management Society of America's (CMSA) model for medication adherence and consists of regular educational mailings and regular individual phone support from an RN Disease Manager to assist with compliance in self-care measures, medication adherence, and reinforcement of physician treatment plans. Participants are also provided with post-hospital follow up reminders in accordance with NCQA’s HEDIS specifications.
