HMO Plans

With Welborn Health Plans HMO, members choose a Primary Care Physician (PCP) from our extensive network of participating physicians. By developing and maintaining a relationship with a primary care physician, employees will receive care from someone they know and trust. With HMO coverage, there are no claim forms to complete, plans available with minimal out-of-pocket expense, and a variety of no-deductible plans (ranging from zero and up). POS/PPO Plans

The POS/PPO products focus on providing members with access to quality, affordable health care, while offering members the freedom to seek care outside the WHP provider network at a reduced level of coverage. Welborn Health Plans POS/PPO plans are cost-effective because we arrange for discounts with our participating network providers and offer greater coverage and access to members who choose them. Wellness Programs

Welborn Health Plans (WHP) believes in wellness & preventive health. To further promote this belief, WHP developed Welcare!. Welcare! is a comprehensive wellness program offered to WHP's employer groups. In fact, we can even customize a wellness program for you and your employees.
 The Welcare! program was created out of a desire to better serve our community's wellness needs. With our wealth of experience, we have developed this comprehensive wellness program, which is facilitated by qualified health professionals.
 This value-added service is yet another example of WHP's commitment to health promotion and wellness.

 INCLUDED IN WHP'S WELCARE! PROGRAM FOR ALL MEMBERS:

  • Finally BEAT Smoking Program:To all members; an alternative and innovative way to help stop smoking. WHP will cover 8 different methods to help our members stop smoking. The cost to the member is no more than their regular pharmacy or specialist office visit co-pay.
  • Health Fair Screenings: Can include screening for total cholesterol, HDL cholesterol, lipid profile, blood pressure, height, weight, body mass index, and blood sugar. Includes education and identification of major health risk factors.
    • Health Achievement Program: Annual individualized Health Risk Appraisal. Individual report mailed to each participant.
  • Gym Discounts: Fitness membership discounts in your area for WHP members.
  • Fit Kids—Fit Families Program: WHP, YMCA, and Welborn Clinic have teamed up to design this program to help kids make better choices in what they eat, and learn to lead more active lives. A parent is required to accompany the child to all classes.
    • Reduced cost for WHP members.
    • Eight-week nutrition and fitness program meets twice a week for 90 minutes, for a total of 16 sessions.
    • Families will receive passes to use the YMCA, 2 additional days per week through the program.
  • Educational Offerings: Optional monthly on-site “lunch and learn” educational offerings regarding health-related topics including back care, healthy heart care, body fat analysis, men’s and women’s health issues, communication skills, healthy eating and nutrition tips, stress, exercise and fitness, and more!
  • CareWise Handbook: A self-care guide for each subscriber.
  • Welcare! "Tidbits" Newsletter: Monthly newsletter covering health-related topics according to annual National Health Observances. Topics introduced in an interesting format that will include heart care, diabetes awareness, nutrition, vision, cancer, exercise, tobacco use, family health, etc.

 

 EMPLOYER-BASED FEE FOR SERVICE:

  • Health Care Advocate: A HCA nurse will spend a designated number of hours on-site at your company each week, time and day to be determined by each individual company. Involving a HCA is most beneficial to corporate wellness programs because of the one-on-one quality of individual counseling those employees receive. Best of all, we would build those one-on-one relationships with your employees to improve their health risks—ultimately with a reduction in long-term health care expenses. Our HCA program is completely confidential.

Third Party Administrative Services (TPA)

Welborn Health Plans offers a full range of services as a third party administrator. Our efficient operations, medical management capabilities, and our dedication to accurate and reliable information processes play significant roles in helping assure the quality and cost-effectiveness of employers' self-funded plans. Health Savings Accounts (HSA)

An HSA is an account that you set up with a qualified HSA administrator, usually a bank, to pay or reimburse certain medical expenses you incur. An HSA is portable, meaning that you can maintain the same account even if you leave your current place of employment or do not remain a member of Welborn Health Plans. HSA balances accrue (without limit) from year to year, meaning that if you do not use all of your available funds in one calendar year, your remaining balance remains available for the next year. Health Reimbursement Arrangements (HRA)

Welborn Health Plans offers Health Reimbursement Arrangements (HRA) accounts in which the employer funds part of the employee’s deductible. This option lessens the impact of higher deductible plans for employees and lowers premium rates for employers. Flexible Spending Accounts (FSA) & Other Ancillary Programs

An FSA is a funding option where employees can set aside tax-free money to pay for qualified expenses.
