Regional Provider Network
| WHP offers an optional participating "Regional" Provider Network though Sagamore Select for certain plans. This network provides the best coverage for providers in the state of Indiana, but outside the WHP service area. Please click on the link to search
for Sagamore Select participating doctors and facilities outside of the WHP service area. |

| Welborn Health Plans (WHP) offers an optional participating "National" Provider Network though MultiPlan, Inc. (PPO). Please click on the link to search
for MultiPlan participating doctors and facilities outside of the WHP service area. |

- Click on "Find a Doctor or Facility" at
the top right corner of the main page.
- Select the correct MultiPlan logo (that
is shown on the back of your Member ID Card).
- Then, simply choose to search for a "Healthcare Facility" or
a "Healthcare Practitioner" to find someone
in your area of interest.
National Provider Network (select plans)
