Welborn Health Plans actively manages the complex and costly care of our catastrophic-case Members. Our Case Management Team is dedicated in assisting our Members in managing their health care benefits to obtain the medically necessary and appropriate care that they require. Identification of the Member's needs is gathered by a Case Management Nurse in order to fully develop a comprehensive care plan.
