Welborn Health Plans is dedicated to helping their members manage their health conditions for a longer, healthier, and more active lifestyle. Our Disease Management Programs are directed at improving health outcomes by providing education, motivation, and support to achieve healthier habits and lifestyles for our members. Programs are designed to educate and influence member’s behaviors to self-care; giving the support and educational materials they need to better manage their disease.
Participation is completely voluntary and at no cost to the member. Easy-to-understand educational materials are periodically mailed to those that choose to participate.
Current Programs:
Heart, Hyperlipidemia & Hypertension Program
Nurse Case Manager Phone Support
Registered Nurse Case Managers are also available by phone to provide additional education and support, if appropriate, depending on the severity of the disease. The Disease Management Case Managers can be reached Monday thru Friday (8am–5pm CST) , so please Contact Us for more information.
