Living with chronic illness is a challenge faced by millions of Americans every day. At Welborn Health Plans, we are there with you every step of the way! Because we know that most chronic conditions respond very well to self-management, we have developed educational programs designed to empower members to take control of their illness. Having a sound understanding of health concerns and learning self-management strategies will lay a foundation for achieving optimal health. Our Disease Management department provides the LINK to Better Health, an educational program offering condition-specific educational mailings and quarterly newsletters designed to address the issues and concerns associated with living with chronic illness.

RN Disease Managers are available upon request, offering one-on-one telephone consultation, answers to health-related questions, and motivation and supports to help you better manage your health and well-being. Disease Management programs currently offered include: diabetes, coronary artery disease, asthma, chronic obstructive pulmonary disease, heart failure, high blood pressure, depression, and migraine. Education pertaining to other conditions is available upon request.
