Healthy-at-Heart (Hyperlipidemia & Hypertension Programs)

Healthy-at-Heart™ is a twelve-month educational program sponsored by Pfizer Inc. that was initiated in 2005. This program is available to both those individuals with Hyperlipidemia and/or Hypertension. This educational program includes a newsletter sent bi-monthly with other promotional items such as measuring cups, pillboxes, etc. In addition, participants receive regular individual phone support from a WHP RN Disease Manager to assist with compliance in self-care measures, medication adherence, and reinforcement of physician treatment plans. At the conclusion of this one-year program, a post-test relating to the participant's disease will be sent. This result will be compared to the pre-test, which participants complete at the initiation of the program. Test results will be evaluated to determine the level of increase in the participants' knowledge and self-management as a result of the program.
