Congestive Heart Failure


Initiated in 2003, the purpose of the Congestive Heart Failure Program is to demonstrate that through participation in the program, plan members diagnosed with CHF would exhibit improved clinical and financial outcomes. The objectives are: to improve the participant’s perceived quality of life; to improve the participant’s functional status, where possible; to decrease the frequency of CHF inpatient admissions and emergency room visits; to decrease the CHF-related costs and overall medical costs; and to improve the participant's compliance with their physician's treatment plan for medications, sodium intake, and weight monitoring and management. Participants receive quarterly educational newsletters and monthly calendars to record their daily weight along with regular individual phone support from a WHP RN Disease Manager to assist with compliance in self-care measures, medication adherence, and reinforcement of physician treatment plans.
