Welborn Health Plans protects the privacy of your information in all settings. When you enroll with Welborn Health Plans, you authorize your health care provider to release your confidential information to your selected health network and Welborn Health Plans regarding a claim, delivery of medical care, and quality measurement reporting. However, no explicit or implicit identifiable information will be released to another organization, including your employer, without your explicit consent, unless allowed by law. In addition, Welborn Health Plans protects your oral, written, and electronic information across the organization. You also have the right to access your medical records, in accordance with Indiana state law.

This notice is mailed to you with your Welborn membership cards when you join.
